By accepting this form you are consenting to put your greatest treasure into our care, which is our honor and pleasure. We will do everything we can to keep your son or daughter safe and sound as we participate in the Burning Hearts Summer Intensive. For each activity we keep in mind safety considerations and emergency procedures so that if anything unfortunate happens we will be ready. However, there are times when unforeseen circumstances beyond our control do occur, and the statements below are intended to protect the congregation and staff of Houses of Light Church in those situations.

In consideration of you accepting my child for participation in Burning Hearts Summer Intensive, I hereby, for my heirs, executors, administrators and myself waive and release any and all rights and claims for damages that I may have against Houses of Light Church and its agents, employees, representatives, volunteers, successors and assigns for any and all injuries suffered by myself or my child that arise out of Burning Hearts Summer Intensive sponsored by the above named organization.

I warrant that I have the right to authorize the foregoing and do hereby agree to hold the above name organization harmless of and from any and all liability of whatever nature, which may arise out of the result from the participation.

For the consideration stated above, I further agree that in the event that my child or I should make a claim against the above organization for damages arising out of the above named program, activity or sport, I will personally indemnify, defend, and hold harmless the organization and its agents, employees, representatives, volunteers, successors, and assigns against any and all loss damage, occasioned thereby, including attorney’s fees.

By accepting the terms, I give permission to treat my child in any case of a medical emergency. By signing below, I also give permission to Houses of Light Church to use photographs, videos, and other recordings, likenesses and images in promoting other activities sponsored by the church.

Al aceptar este formulario, usted acepta poner su mayor tesoro a nuestro cuidado, lo cual es nuestro honor y placer. Haremos todo lo posible para mantener a su hijo o hija sano y salvo mientras participamos en Burning Hearts Summer Intensive. Para cada actividad, tenemos en cuenta las consideraciones de seguridad y los procedimientos de emergencia para que, si sucede algo desafortunado, estemos listos. Sin embargo, hay momentos en que ocurren circunstancias imprevistas fuera de nuestro control, y las declaraciones a continuación tienen la intención de proteger a la congregación y al personal de Houses of Light Church en esas situaciones.

En consideración a que usted acepta a mi hijo para participar en Burning Hearts Summer Intensive, por la presente, en nombre de mis herederos, albaceas, administradores y yo, renuncio y libero todos y cada uno de los derechos y reclamos por daños que pueda tener contra Houses of Light Church y sus agentes, empleados, representantes, voluntarios, sucesores y cesionarios por todas y cada una de las lesiones sufridas por mí o por mi hijo que surjan de Burning Hearts Summer Intensive patrocinado por la organización mencionada anteriormente.
Garantizo que tengo el derecho de autorizar lo anterior y por la presente acepto eximir de responsabilidad a la organización mencionada anteriormente de cualquier responsabilidad de cualquier naturaleza que pueda surgir del resultado de la participación.

Por la consideración indicada anteriormente, también acepto que en caso de que mi hijo o yo presentemos un reclamo contra la organización anterior por daños que surjan del programa, actividad o deporte mencionado anteriormente, indemnizaré, defenderé y mantendré indemne personalmente la organización y sus agentes, empleados, representantes, voluntarios, sucesores y cesionarios contra todos los daños y perjuicios ocasionados por ello, incluidos los honorarios de abogados.

Al aceptar los terminos, doy permiso para tratar a mi hijo en cualquier caso de emergencia médica. Al firmar a continuación, también doy permiso a Houses of Light Church para usar fotografías, videos y otras grabaciones, semejanzas e imágenes para promover otras actividades patrocinadas por la iglesia.
